Understanding AL Amyloidosis Symptoms and Signs of AL amyloidosis Investigations for AL amyloidosis Treatment of AL amyloidosis Troubleshooting for patients taking drugs for AL amyloidosis Darren Foard, Clinical Nurse Specialist at the NAC answers some frequently asked questions about AL amyloidosis:
Immunoglobulin light‐chain amyloidosis (AL amyloidosis) is a rare disease in which a small plasma cell clone produces toxic misfolded proteins that deposit in organs and impair their function. Currently, the only available treatment approach is the elimination of clonal plasma cells. 2021-03-02 · Astellas Pharma GmbH is developing bendamustine for the AL amyloidosis treatment. AL amyloidosis is a burning issue for tens of thousands of patients 1 While there are no currently approved therapies, the consensus is to treat patients with plasma cell-directed therapies with the goal of reducing light-chain production—which in turn can preserve organ function and improve prognosis. 1,2 The most effective treatment is autologous bone marrow transplants with stem cell rescues. However many patients are too weak to tolerate this approach. Other treatments can involve application of chemotherapy similar to that used in multiple myeloma. Thus, not all agents used to treat multiple myeloma shows similar efficacy in AL amyloidosis.
The treatment and prognosis of AL amyloidosis will be reviewed in detail here. The treatment of LCDD and HCDD is also reviewed briefly. The pathogenesis, clinical features, and diagnosis of these disorders and the diagnosis and management of amyloid cardiomyopathy and renal amyloid are discussed in detail separately.
The amyloid deposits cannot be directly removed. But there are treatments to stop more of the abnormal proteins being produced and treat your symptoms. AL amyloidosis treatment is directed towards the abnormal plasma cells (usually in the bone marrow), producing abnormal light chains that form amyloid deposits.

1,2 However, patients with AL amyloidosis are more fragile than multiple myeloma patients and are at a particularly high risk of death in the first few months following treatment initiation [63, 64].
Thus, not all agents used to treat multiple myeloma shows similar efficacy in AL amyloidosis. In AL amyloidosis, both hematologic and organ responses after treatment are important to improve the clinical outcome. Especially, improving heart function is one of the key aspects in the treatment of AL amyloidosis. Some varieties of amyloidosis may lead to life-threatening organ failure. Treatments may include chemotherapy similar to that used to combat cancer.

However, no studies addressed maintenance therapy in AL amyloidosis, and prospective, controlled studies are warranted before it can be considered in this disease. Overall, there is agreement of independent studies showing that organ progression should not be awaited to resume anticlone therapy in relapsing patients with AL amyloidosis. 2021-01-22 · For example, AL, ATTR, and Aβ 2 M can present as carpal tunnel syndrome or gastrointestinal amyloidosis, but each has a different etiology and requires a different treatment approach.
